Comments Sought on Proposed Forms for City and Parish Courts


The Louisiana Supreme Court’s Uniform Forms Committee for City and Parish Courts has completed a set of proposed forms for use by attorneys and self-represented parties in city and parish courts statewide.

The proposed forms are posted online for a 30-day public comment period which began July 12.

Interested parties may comment via email, distctrules@lasc.org; via facsimile, (504)310-2606; or by mail to: Uniform Forms Committee for City and Parish Courts, Louisiana Supreme Court, Ste. 1190, 400 Royal St., New Orleans, LA 70130.

Once the comment period has expired, the Uniform Forms Committee for City and Parish Courts will review all comments, make any changes it deems appropriate and submit a final draft of all forms to the Louisiana Supreme Court for review and approval.
